Kim Russell Smith

Kim has been working in the field of Developmental Disabilities for over 20 years and has been the Administrator at Brookside Extended Care since 2007. Prior to her service at Brookside, Kim served as a Service and Support Administrator (SSA) at the Hamilton County Board of DD as well as created the Constant Companions day program which was one of the first alternatives to sheltered workshops in Hamilton County. This program offered not only day habilitation services but community employment. After partnering with the Hattie Larlham Foundation, the Constant Companions Program became available statewide. Prior to her service in the field of disabilities, Kim worked as an Investigator for the Hamilton County Department of Job and Family Services in the Sex Abuse Unit. Kim has made a commitment throughout her career to assuring the health and safety of the individuals she serves and has become an advocate for individuals who cannot speak for themselves. Kim believes that individuals with disabilities should be given choices and that families should be the primary decision makers for their loved ones when their loved ones cannot make choices on their own. As a former County Board SSA and a current ICF/IID and waiver provider, Kim is committed to assisting families in making informed decisions in regards to care. Kim earned her Bachelor of Science Degree from Xavier University in 1995.

Brookside Extended Care is an ICF/IID that specializes in the care of children and adults with severe to profound developmental disabilities and individuals who are medically fragile. The facility is licensed by the Department of Development Disabilities and receives oversight by the Ohio Department of Health. Brookside offers 24/7 Licensed Nursing care, Physical Therapy, Occupational Therapy, Speech Therapy, a licensed Dietitian, Psychologist, and Case Management Services. Our experienced staff routinely cares for individuals who:

  • Have complex medical needs
  • are G-tube or J-tube dependent or require a modified diet.
  • are Trach dependent or have other respiratory issues
  • require 100% assistance with ADLs
  • require frequent turning and repositioning due to their risk for pressure ulcers
  • are high risk for aspiration pneumonia

Each individual has a case manager (QIDP) and a team of professionals that meet regularly to ensure that each resident’s needs are met. All care that is provided is individualized. Brookside schedules and provides transportation to all medical appointments. The facility communicates and coordinates appointment information with the individual’s guardian.

In addition to Health related services, Brookside offers programs that assist individuals in maintaining skills and developing new skills. Individuals that reside at Brookside are actively involved in their community and participate in a number of activities throughout the year. Children are enrolled in the Mason City Schools and our residents regularly visit the Mason Community Center. Our residents look forward to spring as they have been long time Red’s Season Ticket Holders!

In addition to ICF/IID Services, we are a certified provider of IO, Level 1, and Non-Medical Transportation Waiver Services in Warren County. This allows families a choice of settings with a range of service options.
